Open to youth ages 13-18 with a primary substance use diagnosis

Required documents

Client's full legal name, Social Security number, date of birth, address, and phone number a recent assessment or discharge by a qualified substance abuse professional is required (ASAC can complete this assessment if needed)

Provides a day treatment substance use disorder services to youth ages 13-18. The program offers Partial Hospitalization and Intensive Outpatient levels of care for youth with a primary substance use diagnosis. In addition to group programming, each youth is assigned an individual counselor to meet with weekly.
