Integrated Health Home (IHH) | Bethany for Children and Families

Integrated Health Home services are those services provided to adults with serious mental illness (SMI), and Medicaid coverage.

Services are classified as whole person care, for physical health, mental health, and lifestyle choices that impact overall wellness. Services are provided by an Integrated Health Home Team which includes a nurse care manager, a care coordinator, and peer support specialists.

Integrated Health Home providers help with manage physical and behavioral health services, create a whole health plan for medical care and wellness goals, focus on choices, strengths, good health, and total wellness, help with transportation to healthcare appointments if needed, use technology to link all your services together; and provide peer support services to encourage recovery and resiliency.
