Call for information or appointment or visit for services; Traditional referral sources are a physician or office staff, hospital discharge planner, nursing facility staff, or self-referral; The Hospice nurse will schedule a family meeting and patient assessment

- Medicaid - Medicare - Private insurance - Private payment accepted - Patients without coverage may apply for Medicaid through the Illinois Department of Public Aid - Bereavement services are provided free of charge to anyone in the community that has suffered a loss

Description

Offers care to the terminally ill and their families. Care encompasses the physical, spiritual, and emotional needs of the family unit with emphasis on the desires of the patient and family. Patients are served wherever they call home.

The 8-bed inpatient facility offers a home-like environment, while providing Hospice's comfort-oriented care and services, including:

- Expert pain and symptom management.

- 24-hour nursing care.

- Assistance with tasks of daily living.

- Friendly visits from volunteers.

- Spiritual and psychosocial counseling for patients and their loved ones.

- Bereavement care and support groups for surviving family members.
