Under the direction of a physician, a team of health care professionals will develop a plan of care based on individual needs. Services offered to home health patients include but are not limited to:
- Case Management.
- Telemonitoring.
- Rehabilitation Services.
- Supportive Counseling.
- Pediatric Care.
- Intravenous Therapy.
- Pain Management.
- Wound and Ostomy Care.
- Diabetes Management.
Registered nurses are available 24 hours a day by phone to provide specialty care for patients with virtually any medical diagnosis.
OSF Home Care also offers:
- Diabetes Supply Program which provides comprehensive, convenient supply services and home delivery of diabetes products.
- Home Infusion Pharmacy which specializes in providing home intravenous therapies, clinical therapy management, and other related support services not available from most traditional retail pharmacies.

Call for information; A doctor's order is needed before home health services can begin; Referrals and requests for service are accepted from family members, patients, hospitals, nursing homes, physician offices and clinics, and community agencies
Fee
Medicare covers skilled nursing care, physical therapy, speech therapy, and other services, depending on the individual's needs; Medicare, Medicaid, and other third-party insurances are accepted and handled on the patient's behalf for the Diabetes Supply Program and Home Infusion Pharmacy
