Offers Medical Case Management for people living with HIV/AIDS. Provides connection to low-cost HIV medical care and medication services. Once enrolled, transportation assistance may be available for medical appointments, lab visits, and pharmacy trips. Food vouchers and access to local food pantries may also be available. Referrals to lawyers, legal organizations, and other supportive services can be provided.

Offers programs for those returning home after being in jail or prison, providing care and education about HIV/AIDS, and giving counseling to people who need support.
