Open to individuals ages 65 and older, or 16 years of age before January 1 of the current year and qualify as disabled, must live in Illinois at the time of filing applicant and meet income guidelines

Previous year's tax return, All W-2s, Social Security 1099, Pension statements, Social Security card, initial letter stating the that the applicant was classified as disabled (for those filing as such), Bank statements cannot be accepted

Paper applications are not available. Application may take up to 8 weeks to process. May check application status 48 hours following submission. Once application is approved, print the certificate of eligibility and submit to the local transit authority or Secretary of State's Office. Wait 10 business days from the date of approval for license plate discount to be available. If ride free benefit was requested, contact the local public transit system for further information.
