Description

Assists adults and children who have mental health disorders with support and guidance towards improving their overall health. Program focuses on an individual's whole-health and needs. Individual needs may include goals related to physical health, mental health, emotional health, spiritual, social, and community related goals. The IHH team also works with healthcare providers to coordinate care, achieve health goals, and improve wellness.
