Must be a McHenry County resident, ages 55 and older, with no insurance or other means of paying for needed dental work, assistance is limited to $1000 per person, the dentist must be willing to accept the voucher and send the bill to MCHA for final payment
State issued photo ID, social security cards for all household members, all documentation of gross income for the past 90 days for all household members, current bank statements and proof of receiving assistance from other sources within the past 90 days

Description

When funding is available, assists those who are at least 55, are income eligible, and who have no insurance or other methods in which to pay for needed dental work. Applicants must be experiencing current dental pain and/or have difficulty eating/chewing, or have missing/broken and/or decayed front teeth.

The program uses a voucher system and allows recipients to receive treatment from a provider of their choice. Cannot pay for dental work completed before the date of MCHA's dental voucher. This program is funded by The McHenry County Senior Services.
