- Photo ID
- Proof of income (for the last 30 days)
- Current Identification for those living in the household (medical card, school records, DHS statement, etc.)
- Current piece of mail for proof of address A country ID or complete medical card printout is acceptable
A state-issued medical card is sufficient for address and income
A driver's license does not count as a form of address verification
Documentation will be required for both in-person appointments and online registration
