Provides door-to-door transportation for eligible individuals with regular, scheduled transportation needs, such as transportation to work or medical appointments. Transportation is limited and provided based on availability. One-time or on-call trips, such as transportation to a single medical appointment, are not provided. Most clients must have a referral from a case manager and be approved before transportation can begin. Eligibility requires a Medicaid waiver for a disability, brain injury, or age-related services.
