EMERGENCY SUPPORT PROGRAM | CHI HEALTH GOOD SAMARITAN
Coordination of follow up care for individuals who are experiencing a substance abuse or mental health crisis, including making appointments with counselors, community support workers, physicians, and other appropriate entities. Provides ongoing support and advocacy for 30 to 120 days, based on individual need.
