Description

Program for adults with a chronic mental illness aimed at achieving mental health stability within the community. Services may include medication management and monitoring, symptom monitoring, coordination and development of supports, crisis planning, communication with other professionals, and peer mental health support services. Peer support providers with lived experience, trained and certified to assist individuals, may help develop and implement personal recovery plans, model effective coping techniques, support self-advocacy, and complement traditional clinical care as part of an evidence-based mental health model.
