SS cards or ITINs for all household members, photo ID, copy of prior year tax returns (state and federal, especially Iowa residents), all W-2 and 1099 forms, child care provider information, health insurance documentation, bank account and routing number (for direct deposit), gambling winnings forms, education expense receipts, and any other applicable documents
