FAMILY CASE MANAGEMENT | MOULTRIE COUNTY HEALTH DEPARTMENT
A program that assists pregnant women and children to obtain health care services in order to have a healthy pregnancy and promote the healthy development of the child and family. The goals of the program are:
-- To provide access to primary health care.
-- To identify and resolve access barriers through referral and linking.
-- To provide health education to all eligible clients.
-- To reduce infant mortality.

Open to pregnant women and infants in families (most pregnant women and infants enrolled in the Women, Infant and Children nutritional program are also served by Family Case Management), High Risk Infants (infants born with medical complications who spend time in a neonatal intensive care unit), and foster children ages 6 and under; Must meet income requirements
