Must be a burn survivor 18 years or older and take at least 4 credit hours maintaining a minimum GPA of 2.5 (on a 4.0 scale)

Proof of acceptance into an accredited institution, A statement of reason to pursue training or schooling, A medical professional's statement (description of injuries, dates, medical care, and progress), A completed application form for each semester, Any other information the applicant feels should be included
