Eligibility
Open to adults age 18 and older with an intellectual or developmental disability who have completed a member enrollment or annual member renewal
Required documents
Proof of intellectual or developmental disability Proof of registration on PUNS list (done through Prairieland Service Coordination) or proof of Medicaid Developmental Disabilities Waiver services Proof of residence Community Choices may help obtaining required documents if clients do not have them
