Provides Supported Community Living services for people with Intellectual, and or Mental health diagnosis, under Habilitation, and also ID waiver. Services are provided within the member’s home and community, and in a variety of community-based, integrated setting. Members may live in the home of their family or legal representative or in other types of typical community living arrangements. SCL services are intended to provide for the daily living needs of the member and shall be available on an as needed basis up to 24 hours per day.
