Description

Imagine the Possibilities provides home and community based services. This is a Medicaid program designed to assist both children and adults with disabilities live in and become vital members in their communities. This program allows individuals with disabilities to receive precisely the services they need to enable them to live in a setting of their choice and be as independent as possible.
