Provides in-home assessments which help identify the needs and strengths of each individual and develops a client-directed care plan, that promotes independence and prevents premature institutionalization.
Resources and supportive services may be arranged, such as Homemaker Assistance, Adult Day Services, Emergency Home Response Services, and other programs. The goal is for individuals to make informed choices regarding their own care.
