Required documents
Birth certificate (U.S. Hospital record if no birth certificate), driver's license or state-issued non-driver license identification card, if you have none of the prior information please bring employee identification card, school identification card, health insurance card, or U.S. Military Identification card, all cards must be current, show name and identifying information with a picture
