CHILDREN'S GROUP HOME PROGRAM | GOLDIE B. FLOBERG CENTER

Serves children and young adults with developmental disabilities in community integrated, 4 person group homes.

Children receive 24-hour support and supervision. Nursing, physical therapy, psychiatric, and behavior management services are also offered.

Activities for children to participate in include Boy and Girl Scouts, Therapeutic horseback riding, school activities, going to movies, museums, zoos, hopping, and other community activities.

Application process

Referral required by the Illinois Department of Human Services, referrals to IDHS can be made by anyone

Must receive a Medicaid 1915(c) waiver

Eligibility

Open to ages 5-21, both ambulatory and non-ambulatory children are served. Program focuses on serving children and young adults with a primary diagnosis of Autism and/or any intellectual disability, a secondary diagnosis involving emotional or behavioral issues is also acceptable
