The agency will contact the treating physician by fax to make sure it is safe for the client to fly Include the name and fax number of physician The pilot will ask to view I.D. to verify identity Client will be asked to read and sign a release form prior to boarding each plane

Visit website and click on the "passengers" menu tab, read through the qualifying information, watch a video to see what it is like to ride in a small, non-pressurized aircraft, and learn about AFC; To submit a flight request, click on "Request a Flight" and fill out the questions - a Flight Coordinator will contact client once the application is submitted
