The Wellington Place provides one bedroom apartments with a month-to-month lease with various levels of assisted living and skilled nursing and personal care services. Services include emergency call system; weekly laundering of bedding; light housekeeping; personal care and chore assistance; nutritious meals ( lunch, dinner); secured building; utilities paid with the exception of personal telephone and cable TV; individual temperature control; access to intermediate/skilled nursing; and on-site occupational therapy, physical therapy, and medication assistance.

Operates eight intellectual and developmental disability community homes (group homes) which enable an adult to live in their own apartment with support and assistance. Staff help individuals build independent living skills such as budgeting, grocery shopping, and housekeeping. Also provides psycho-social support to help participants cope with daily stresses.

Community homes provide a family living environment where adults share a home in the community with up to seven peers, under staff supervision. The residents share in all aspects of running a group home, such as cooking, cleaning, and grocery shopping.

Assisted residential services provides structured supervision, evaluation, and training, independent living and social/recreational skills as prescribed by an Individual Program Plan (IPP). Homes are located in residential neighborhoods where clients are active members of neighborhoods and communities. Staff is available to provide 24-hour supervision and training if needed.

Provides quality, affordable and accessible housing to people with intellectual and developmental disabilities. People who apply to live in Mosaic housing do not have to receive other services from Mosaic and may receive services from other providers. All residents must qualify according to established income guidelines. Income must be below the low or very low-income guidelines for the county of residence.

Services for people with an intellectual disability and who may also have a physical disability. The program consists of Residential Care Facility-Intellectual Disability (RCF-ID) licensed group homes housing eight individuals with intellectual or developmental disabilities. Staff provide supervision, instruction, and assistance in community living. Residents receive 24-hour supervision and assistance and must participate in daily work or day habilitation services.
Eagle Ridge is an 18 unit assisted living complex. Amenities include: one and two-bedroom units or suites with full kitchens, accessible bathrooms, thermostat, storage unit, and private mailbox; on-site, 24-hour nursing staff; medication administration; meal plans available; housekeeping and laundry services; transportation services; planned social and recreational activities; wireless call pendants; community dining and living rooms; on-site beauty and barber shop; and a whirlpool spa.

Inspiring Lives offers a 24-hour residential care facility including efficiency apartments, suites, private, and semi-private rooms. Treatment services consist of individual program planning through team process with counselor/therapist and consulting psychiatrist (on-site service). Social activities, such as, bingo, parties, shopping, exercising, plays, picnics, dances, sing-a-longs, Bible study, church/mass services, swimming, fishing, etc. are available. Daily living skills instruction includes address personal hygiene, housekeeping, money management, laundry, cooking skills, behavior modification, etc. Also includes on-site RN/LPN services consulting in podiatry, family practice, and psychiatry; a doctor on-call; individual and group classes (2 days/week) in a variety of subjects; individual and group counseling and coordination with local providers of substance abuse services to provide on-site services for clients three days/week.

YOUTH COMMUNITY SERVICES offers programs for children with autism and works to meet their diverse needs. ADULT COMMUNITY SERVICES program for adults with autism uses an individualized treatment plan based on the client's needs. Services can range from intensive 24-hour supervision to 1-2 hours of support per week. Accept Medicaid waivers, habilitation, state/county, and private pay.
