Outreach health care and health promotion/disease prevention to residents of Madison County. Includes home, hospital, and office visits; health and safety education; transportation in Norfolk and to/from Santee and Winnebago Indian Health Services Facilities for appointments; making appointments; and calling in and delivering prescriptions. A 24 hour notice is required for transportation.

Provides medical transportation to the Shriner Hospitals in Chicago, Cincinnati, Philadelphia and Minneapolis for children who need to go to these hospitals.

Provides transportation to and from non-emergency medical appointments, grocery stores, food banks and farmers markets.
Helps Iowa families with children and teenagers (birth through 20 years) find medical, dental, vision, and mental health providers; schedule health care appointments; connect to community resources; screen to see how young children are learning and growing; and enroll children in Hawk-I and Medicaid health insurances.
Provides a limited number of rides to senior citizens. Must call one day in advance; Limited to 6 round trips or 12 one way trips per fiscal year July 1st - June 30, Transportation is on a case by case basis and limited to trips for medical appointments and pharmacy pick ups/drop offs.

Provides medical appointment transportation for seniors. Other transportation needs are not covered at this time. Transportation is available for trips both within the county and also into surrounding areas outside of the county.

Provides professional and personalized non-emergency medical transport for the elderly and disabled in the Greater Des Moines Metro.

Medical includes doctor's appointments, non-emergency hospital visits, hospital discharge, dialysis, physical therapy and stroke rehabilitation, pulmonary and cardiac rehabilitation, and dental appointments.

Non-Medical includes long-distance trips, airports, train or bus stations, special and family events, sports events, and more.

Provides transportation to residents of Unincorporated Saint Charles Township, who are disabled or over the age of 65. The riders may ride within Kane County for 3 round trip rides per week or 6 one ways. The purpose of these rides may be for medical, dental, therapy or chiropractic appointments, a job, to the Elderday Center or to School.

Transportation Expense Assistance

Provides transportation assistance for medical, dental, and mental health appointments to people living with HIV/AIDS in accordance with guidelines.

Available for any area in the 24-county region that is not served by public transportation.

Volunteers provide transportation for medical purposes, to access food resources, pharmacies, libraries, church and more to eligible community members.

Volunteers can serve at HIRTA Helps' volunteer stations, which are local public agencies and nonprofits. Volunteers receive mileage reimbursement and supplemental and liability insurance.

Provides transportation to a variety of destinations including medical appointments to hospitals and clinics in Delaware, Dubuque and Jackson counties. Also provides transportation to the University of Iowa Hospitals and Clinics.
