Services include blood pressure screening, case management for the Frail Elderly, communicable disease control, care for yourself program (cancer screening program), emergency preparedness planning, immunization clinics, communicable disease control links, and Iowa Department of Public Health Centers for Disease Control and Prevention.

Health education is integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care, and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Provides mammograms and Pap tests for early cancer detection, lead poisoning screenings for young children, and home health aide services to support recently discharged hospital patients, elderly individuals, and people with disabilities. Works closely with the county Board of Health and serves as a funding source for community needs assessments. Also provides information about communicable diseases.
