Health center for care of acute and chronic illness. Transportation available for appointments at the clinic. Services can include breast exams and pap smears, cholesterol screening, diabetes management, immunizations, flu, and pneumonia shots, infant checkups, pregnancy testing, physicals for school, sports, and work, and sexually transmitted disease tests and treatment.

Provides public health services to the residents of Boone County. Services include blood pressure screening, case management for the Frail Elderly, communicable disease control, care for yourself program (cancer screening program), emergency preparedness planning, immunization clinics, communicable disease control links, and Iowa Department of Public Health Centers for Disease Control and Prevention.

Health care for all ages, including obstetrical, pediatrics, geriatrics, general medical care, women's health, reproductive health, prenatal, obstetrics and gynecology, and radiology/ultrasound.

Blood testing for lead.

HIV testing and counseling.

Tuberculosis prevention and case management.

Immunizations for children and adults, including flu vaccines. COVID vaccines available in office or at home.

Screening and treatment for cardiovascular diseases, diabetes, cholesterol, and depression.

Routine services such as pap smears, school physicals, prenatal visits, well baby exams, physicals and other routine healthcare procedures. Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns, and other on-going health problems are also routine activities at the clinic.

Health education is integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care, and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Offers public health programs and services, including bioterrorism and county disaster preparedness. Provides mammograms and Pap tests for early cancer detection, lead poisoning screenings for young children, and home health aide services to support recently discharged hospital patients, elderly individuals, and people with disabilities. Works closely with the county Board of Health and serves as a funding source for community needs assessments. Also provides information about communicable diseases.

Offers healthcare services for all stages and aspects of a woman’s life. Services include pap smears and screening tests, contraception and family planning, menopause management, and gynecologic surgery (minimally invasive surgery).

Also provides information regarding communicable diseases. Offers the following programs for expectant parents and new parents: HOPES Program, 1st Five, Early Head Start and the Maternal Child Health program. Immunizations for children, adolescents, and adults along with the flu vaccine.

Provides clinical breast exams, mammograms, pap tests, and pelvic exams for qualifying women.

Provides free beast and cervical cancer screening to eligible women 21 years and older.

Comprehensive reproductive health center providing prenatal and family planning (birth control) services. Other services include free pregnancy testing for women of any age, testing/treatment of sexually transmitted diseases for males and females, hormone replacement therapy, sterilization, education programs on sexual responsibility, female camp, school and sport physicals, and annual examinations for adult women. Bus tokens may be available for those with transportation needs.

Currently enrolling Iowa women in a program that provides free birth control services to those who have no insurance and meet income qualifications. Can provide long acting reversible birth control to any woman who medically qualifies for little to no out-of-pocket expense.

Teen pregnancy prevention program addresses questions relating to sexuality issues such as pregnancy, abstinence, birth control methods, and sexually-transmitted diseases. Anyone may request a free Teen Pack or Parent Pack, which include information on teen parenting, communication, birth control methods, and sexually transmitted diseases.
