Offers programs focusing on disease and injury prevention for older adults and caregivers.

Clinic Programs: Provides comprehensive foot care, health assessments, health screenings (which include blood pressure, cholesterol/triglycerides, glucose, bone density), ear wax removal, medication, and safety checks to inform participants about personal health risks and the things they can do to lower risks and optimize health.

Health and Fitness Programs: Offers fitness and health education classes and physical activity programs, conducted for community, church, and social groups. Programs cover the areas of illness and injury prevention, physical activity, nutrition, and mental wellness. Individuals are encouraged to take measures that decrease risk, promote optimum health, and encourage independent living.

Routine services such as pap smears, school physicals, prenatal visits, well baby exams, physicals and other routine healthcare procedures. Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns and other on-going health problems are also routine activities at the clinic.

Health education integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Health screening programs offers regular testing for breast and cervical cancer, heart disease, and diabetes screening for women ages 40-74. Includes pelvic exam, pap test, clinical breast exam, screening mammogram, blood pressure check, cholesterol check, blood sugar check, and health and wellness information.

If there is a problem (such as a breast lump or an abnormal Pap test) that needs to be diagnosed, or to find out the cause, breast and cervical diagnostic services may be available for women ages 18-74.

State Pap Plus program offers STD screening (age 18+), pap tests (ages 21-29), and cervical cancer screenings (ages 30-39).

The program also offers colon cancer screening tests to Nebraska men and women who are 50 years and over, who meet income and insurance guidelines.

La Clinica De La Esperanza provides medical services in the areas of adult, pediatric and women's health. La Clinica also provides a registered nurse to teach educational classes in Spanish on a variety of health-related topics including prenatal care, breastfeeding, family planning, nutrition and diabetes. An on-site social worker is also available to clinic patients for therapy in a clinical setting.
