Routine services such as pap smears, school physicals, prenatal visits, well baby exams, physicals and other routine healthcare procedures. Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns, and other on-going health problems are also routine activities at the clinic.

Health education is integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care, and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Mercer County Health Department

BBO-C is a statewide program (outside the City of Chicago) that provides pregnant and parenting clients with direct, comprehensive, nursing assessments and assistance to connect and engage with desired services and understand the importance of a medical home.

Nurse Navigator's provide nursing assessments and any immediate nursing interventions necessary to dyads (pregnant and postpartum individuals and their infants), helping to connect families to the needed supports available in their community during in-person visits. Nurses are required to conduct the initial pregnancy visit and initial postpartum visit for this program and are expected to follow up as indicated using clinical discretion. Non-nurse navigators may provide follow-up at the discretion of the nurse for collaborating with each family to identify and mitigate barriers to accessing desired supports and services.

Community Health Care - Edgerton Clinic

Comprehensive reproductive health center providing prenatal and family planning (birth control) services. Bus tokens may be available for those with transportation needs.

Currently enrolling Iowa women in a program that provides free birth control services to those who have no insurance and meet income qualifications. Can provide long acting reversible birth control to any woman who medically qualifies for little to no out-of-pocket expense.

Teen pregnancy prevention program addresses questions relating to sexuality issues such as pregnancy, abstinence, birth control methods, and sexually-transmitted diseases. Anyone may request a free Teen Pack or Parent Pack, which include information on teen parenting, communication, birth control methods, and sexually transmitted diseases.

Services include help locating a doctor or dentist, finding health care coverage -Title XIX or Hawk-i, reminders for child/children medical and dental checkups, making doctor appointments, help to find community resources, health education and support, oral health services, help with payments for physical and dental exams, blood lead testing, immunizations 0-6 years, 7-18 years and adult immunizations.
