Community-based services and support to persons of all ages with developmental disabilities.

Residential program includes staffed group homes for youth and adults, supervised apartments, and independent living arrangements in Alliance, Chadron, and Hay Springs.

Vocational training and employment services through a supervised workshop, Niobrara Enterprises, includes on-site contract work, work stations in industry, supervised employment programs, and job coaching.

Year-round educational program is available to children through contractual arrangements with the school district.

Adult leisure and recreational center for individuals who have reached retirement age. Clients are served through an alternative day program that emphasizes recreation, leisure, and maintenance of daily living skills. Alternative day services are also available for other adults whose needs do not include vocational or employment goals.

DAY HABILITATION PROGRAM | IMAGINE THE POSSIBILITIES

Services are provided to individuals who would like to work on life-skill goals such as cooking, cleaning, socialization, community safety, etc. through a program called Community Integration. Individuals have the opportunity to be active and access the community through communication and socialization activities.

COMMUNITY SERVICES | OPTIMAE LIFESERVICES - CENTRAL IOWA REGION

Offers the following Community Services:

Home and Community-Based Services (HCBS) provides services for adults with intellectual or mental disabilities, a qualifying brain injury or handicapping condition. Individualized support is provided in the client's home or other community-based settings and is available 24 hours.

Home-Based Habilitation Services (HBHS) are designed to assist individuals with serious and persistent mental illness with the acquisition, retention or improvement in skills related to living in the community. Services assist in the daily living needs of the client and can be provided 24 hours a day.

Community Support Services (CSS) provide integrated services and supports to persons with a serious mental illness. Interdisciplinary teams provide assessments and service planning, monitoring of mental health symptoms, medication management, problem-solving assistance, coordination of appointments and transportation and support.

Provides developmental programs of assessment, treatment, training, instruction, care, rehabilitation, and support for persons with intellectual disabilities and other developmental disabilities. Provides respite care on-campus or in-home. Provides consultation, training, assessment, and information to support agencies and individuals serving people with disabilities in the community. Offers parent training.

COMMUNITY DAY SERVICES | WARREN ACHIEVEMENT CENTER

Provides community day services, including four classrooms for those living with cognitive and developmental disabilities. Participants are taught life skills, nutrition, hobby development, social interaction, conflict resolution, anger management, exercise/fitness, and more.

Activities include monthly parties, day camps, field trips, gardening, volunteering/community integration, and more. There are two full-time nurses on staff that manage a wide variety of duties.

Life Club takes place on site at Camp Sunnyside as well as in the Des Moines area community. Life Club clients make friends, get out to see different community attractions, choose and lead activities they enjoy. Team members focus on strengths and independence as clients participate in activities.

Clients Socially Integrated is a community-based day program for adults with disabilities focused on learning transferable skills that lead to independence in the community. This is achieved by spending the day out in the community, learning to navigate the public transportation system, relationship building, problem solving and deciding as a group what activities to do. Communication, safety, transportation and independent skills are sharpened as clients visit historical sites, volunteer, visit coffee shops, go to movies and much more.

Connections provides comprehensive support services aimed at fostering coping, problem-solving, and self-management for individuals with diagnosed mental illnesses. The center offers morning refreshments, including coffee and doughnuts, as well as a midday meal. The program emphasizes daily living skills, recreational and social activities, and developmental support to help participants enhance functional skills for community involvement, self-advocacy, and self-care.

Activity rooms are available for recreational and social engagement or relaxation, providing opportunities for personal and social development in a structured environment.

Connections also maintains a donation room stocked with clothing, shoes, coats, gloves, and other essentials, updated frequently to meet client needs. Staff members assist with housing searches, accessing medical benefits, and connecting clients to additional services and resources.

Offers two levels of Supported Community Living Services designed to meet the daily living needs of adults with disabilities and mental illness.
