Designed to help individuals optimize their personal and social competency to live successfully in the community. Activities include volunteerism, leisure and recreation, health and wellness, personal empowerment, access to the arts, making friends, pursuing hobbies, passions, and more.

Offers supported employment opportunities to individuals who have both the desire to work in an integrated work setting and employment abilities for this setting. The two main goals of these services are to give the individuals the opportunity to earn competitive wages and allow these wages to be earned in an integrated work setting.

Provides vocational training for persons who require a higher level of care than is available through other Hope Haven programs. Individuals receiving services must have a primary diagnosis of a mental disability and must be certified by the Iowa Foundation for Medical Care to be in need of this level of service.

Home and Community-Based Services (HCBS) provides services for adults with intellectual or mental disabilities, a qualifying brain injury or handicapping condition. Individualized support is provided in the client's home or other community-based settings and is available 24 hours.

Home-Based Habilitation Services (HBHS) are designed to assist individuals with serious and persistent mental illness with the acquisition, retention or improvement in skills related to living in the community. Services assist in the daily living needs of the client and can be provided 24 hours a day.

Community Support Services (CSS) provide integrated services and supports to persons with a serious mental illness. Interdisciplinary teams provide assessments and service planning, monitoring of mental health symptoms, medication management, problem-solving assistance, coordination of appointments and transportation and support.

Provides supported employment services for individuals with disabilities to obtain and maintain community employment. 

Offers day habilitation services that are provided outside a member’s residential setting and are designed to foster the acquisition of skills, building positive social behavior and interpersonal competence, greater independence, and personal choices. Activities include assistance with acquisition, retention, or improvement of self-help, socialization and adaptive skills that enhance social development and offer opportunities for community inclusion.

Provides day habilitation and community integration services to build and improve skills for daily living, self-care, social skills, and community living.

Provides support and training through vocational, residential, and day services to persons with developmental disabilities and/or related conditions.

Offers two levels of Supported Community Living Services, to meet the daily living needs of adults with disabilities and mental illness.

- Programs are designed to increase a client's independence and provide a level of relief for family members and caretakers. The level of service is determined by the individual, guardians, case managers, and providers to best suit their needs.

- Services may include employment assistance, day habilitation programs, and more.

Provides access for people with disabilities to spend time in the community and build social skills. Activities include learning how to have conversations, manage feelings, budget money, and make purchases. The program offers support to help people learn how to get around and take part in community activities with less help while building confidence and relationships.
