Offers opportunities to meet new people and explore new places. Helps members navigate the complex world of friendship and connection. Social Coaches work with individuals and groups to build skills that create fulfilling connections. Membership is required.
-- Social Opportunities: Provides weekly events where members can connect with other members with disabilities. Staff support members as much as they need. The purpose of social opportunities is to meet other people and explore community spaces.
-- Co-Op Meetings: Provides quarterly information sessions to members with disabilities and their families (who are also members). Provides information about disability services in the area and how to better utilize Community Choices services and programs. Co-Op meetings also include a 30-minute social time for everyone to connect and catch up with the cooperative community.

Home and Community-Based Services (HCBS) provides services for adults with intellectual or mental disabilities, a qualifying brain injury or handicapping condition. Individualized support is provided in the client's home or other community-based settings and is available 24 hours.
Home-Based Habilitation Services (HBHS) are designed to assist individuals with serious and persistent mental illness with the acquisition, retention or improvement in skills related to living in the community. Services assist in the daily living needs of the client and can be provided 24 hours a day.
Community Support Services (CSS) provide integrated services and supports to persons with a serious mental illness. Interdisciplinary teams provide assessments and service planning, monitoring of mental health symptoms, medication management, problem-solving assistance, coordination of appointments and transportation and support.

Year-round educational program is available to children through contractual arrangements with the school district.
Respite care may be available, call for details.

Clients are served through an alternative day program that emphasizes recreation, leisure, and maintenance of daily living skills. Connections has various activity rooms available for those seeking activities to participate in or just a common area to relax and unwind during your time at the center. Connection does have a donation room available for those in need of clothing, shoes, coats, gloves, etc. This room and supplies are update frequently.
Connections staff can offer support and assist with finding housing, assistance with getting medical benefits and offering/setting up additional services/resources clients may otherwise not know about.

Activities include monthly parties, day camps, field trips, gardening, volunteering/community integration, and more. There are two full-time nurses on staff that manage a wide variety of duties.
