Helps veterans who have physical and mental health challenges find and keep jobs through three different services:

Transitional work program - matches each veteran’s skills with a specific job and provides supervision to help them gain work experience.

Supported employment program - provides long-term support for veterans with serious mental illnesses or physical disabilities to help them compete for jobs in their communities.

Vocational assistance program - trains veterans living at the VA residential facility to improve job search and career development skills.

Supports involvement at work and in the community through habilitative community inclusion, habilitative workshop, prevocational services, and supported employment services.

Provides disability services, including intermediate care facility for individuals with intellectual disabilities, daily and hourly home-based habilitation and support community living, day habilitation, and supported employment.

Offers a Disability Access Point, or "DAP", which helps individuals with disabilities, their families, and their caregivers find the services and support they need-both short-term and long-term. DAPs help by giving information, making plans, making referrals and checking in to make sure people are getting the right support to live healthy, independent lives.

Can help with the following services:

- Information and Assistance - Based on presented and identified needs, provides information about programs and services, and helps to connect individuals to service providers in their communities.

- Options Counseling - Provides guidance to individuals so that they may make informed choices about supports and services, typically over a period of 90 days or less. This includes benefits screening and application assistance for programs and services and follow-ups to make sure supports and decisions are assisting the individual.

- Short-term services and supports (STSS) - Include adaptive and assistive equipment, home and vehicle modifications, peer and parent support services, transportation assistance, and other basic needs (time- limited, not covered by insurance). For STSS, individuals will engage in a short-term person-centered plan around services and supports.

- Long-term services and supports - Includes adult day programs, consumer-directed attendant care (CDAC) (non-skilled and skilled), day habilitation, employment like job skills training, prevocational services, and supported employment, intensive residential services (IRSH), personal response systems, respite services like in-home, in-facility, and day camps, supported community living (SCL) and home-based habilitation (hourly), and residential living settings.

Offers training to people with disabilities in manufacturing, retail, and janitorial jobs. The retail program takes place at Affordalbes and Market Place Thrift Shops and teaches how to use a cash register, stock shelves, and help customers. the manufacturing program includes making and reusing items for sale in the thrift shop.

Offers supported employment opportunities to individuals who have both the desire to work in an integrated work setting and employment abilities for this setting. The two main goals of these services are to give the individuals the opportunity to earn competitive wages and allow these wages to be earned in an integrated work setting.

Provides vocational training for persons who require a higher level of care than is available through other Hope Haven programs. Individuals receiving services must have a primary diagnosis of a mental disability and must be certified by the Iowa Foundation for Medical Care to be in need of this level of service.

The level of service is determined by the individual, guardians, case managers, and providers to best suit their needs.

Offers two levels of Supported Community Living Services designed to meet the daily living needs of adults with disabilities and mental illness. These services increase a client's independence and provide a level of relief for family members and caretakers.
