Provides services coordination/case management to those disabled and/or older adults who meet nursing facility level of care for the Medicaid Waiver Program. This program is based upon the belief that people with care needs should have options for receiving services. Potential participants will meet with a staff person to determine needs and develop a care plan. Eligible persons are offered the choice of receiving home and community-based services or nursing facility care.

The Waiver allows Medicaid money to be used to purchase the following services that are not usually considered 'medical': Personal Care, Companion Care, Chore Services, Adult Day Services, Independence Skills Management, Personal Emergency Response System, Respite Care, Assisted Living Services, Assistive Technology and Home Modifications.

Locates and pays for medical services to children with major medical problems. Child must be medically and financially eligible. Diagnostic services can be considered.

Services includes payment for medical care, hospitalization, specific equipment, and other services. Potential Disabled Children's Program services include transportation, lodging, and respite care.

Financially assists individuals so that they can remain in the most appropriate living arrangement for their individualized needs. Eligible clients can reside in their own home, an apartment, an assisted living facility, a licensed mental health facility, a center for the developmentally disabled, a room and board situation, or in a nursing home.

The AABD program can also, in certain circumstances, provide essential items to qualified recipients such as guardian/conservator fees, home repairs, furniture, appliances, etc.

Medicaid Waiver in-home services, including assisted living, for those age 65 and older with care needs. Must be eligible for Medicaid.

Must be Medicaid eligible, with needs which would require nursing facility care and can be safely served at home. May include services coordination, in home help (home delivered meals, laundry, chore services, personal emergency response, assistive technology, etc.), respite care, independence skills building, child care, community supports (adult day services, transportation for medical appointments), or assisted living services.

Provides case management and social work services for older adults ages 60 and older. Services include community services or resources to help meet their care needs. Care managers arrange for a home visit to meet the client in their home environment. Care planning begins with a comprehensive assessment of each individual's needs, abilities, circumstances and support.

Provides a program that allows Home and Community Based Services (HCBS) Medicaid waiver program recipients the option of finding and hiring their own in home care. CDAC in home care providers can offer either skilled services (medical in nature) or unskilled services (normal daily life activity). All CDAC Agreements must be approved by the Medicaid members case manager or Department of Human Services service worker. TO BECOME A CDAC PROVIDER - contact the Iowa Medicaid Enterprise and request an application to become an individual HCBS CDAC provider (Provider Enrollment Department). The IME will mail an application form with instructions for filling it out and request a background check to be done.
