Intermittent services to maintain or increase independent living skills. For people who live in their own apartments, staff provide personalized supports including assistance with banking, grocery shopping, medical appointments and accessing their communities. The amount of support is guided in partnership with staff and is documented in the Individual Program Plan. Also provides respite services as an additional support to families. Typically, people served are eligible for Supported Residential Services if they do not require staffing 24 hours a day, seven days a week.

Provides HCBS Intellectual Disability and Brain Injury Waiver Community Living and employment supports to adult age 18 and up. Candeo now offers outpatient mental health therapy to children and adults. Services include:

COMMUNITY LIVING SUPPORT for individuals with disabilities including in-home and community supports that assist individuals with disabilities to live independently in the community of their choice. Serves individuals with autism, intellectual disability, developmental disabilities, brain injury, individuals dealing with interfering behaviors and dual diagnosis of intellectual disability and mental illness.

EMPLOYMENT SUPPORT for individuals with disabilities. This service provides community supports to find, obtain, learn and maintain a competitive employment position in the community. Specializes in individuals with intellectual disability, developmental disabilities, brain injuries and chronic mental illness.

HOME BASED HABILITATION SERVICES are for individuals who have a mental illness. Provides supports in the home, community and employment site to assist the individual to maintain independent living and employment.

MENTAL HEALTH THERAPY for children and adults of all ages. Candeo therapists specialize in Art and Play Therapy, Trauma recovery and more. The team also specializes in therapeutic support for adults experiencing both an intellectual disability and a mental health diagnosis.

OWL is an approved provider for the Brain Injury Waiver Program, which was created by Iowa State Legislature in 1996. Services provided through the program include, supported Community Living-designed to assist the consumer with daily living needs, which includes, but are not limited to, community skills, personal needs, budgeting, social skills, home management and individual advocacy.

Provides Supported Community Living services for people with Intellectual, and or Mental health diagnosis, under Habilitation, and also ID waiver. Services are provided within the member's home and community, and in a variety of community-based, integrated setting. Members may live in the home of their family or legal representative or in other types of typical community living arrangements. SCL services are intended to provide for the daily living needs of the member and shall be available on an as needed basis up to 24 hours per day.

HCBS (HOME AND COMMUNITY BASED SERVICES) programs provide training and support to people living in shared homes in the community with opportunities for learning skills to help develop independence.(Pella/Knoxville and Indianola/Des Moines regions).

SCL (SUPPORTED COMMUNITY LIVING) program is for those people who just need someone to "drop-in" to assist them with various things. Individuals in this program can live with their families, or they can live in their own home or apartment. A Living Skills Advisor will schedule time to meet with them every week to assist them with needed tasks - budgeting, cooking, cleaning, shopping, appointments, social skills, and problem solving.

SPIRITUAL SERVICES are available to all clients.

Independent Living Skills Instruction

Supported Community Living and Home Based Community Services to provide the support necessary to assist adults with severe and persistent mental illnesses to live, work and enjoy recreational opportunities in community settings and live as independently as possible.

Community Support Services include monitoring mental health symptoms, transportation, supportive relationships, communication with other providers, assistance in attending appointments and obtaining medication, and crisis intervention.

Day Habilitation programs provide daily structure and positive support to help clients establish relationships.
