Provides community and street outreach services to assist individuals who may be facing homelessness by working to increase job, social, and living skills. Will also assist individuals with obtaining benefits (Social Security, Medical, etc.) for which they may qualify.

Provides a safe place for at-risk and homeless youth (age 16-24) to connect with services. Services include street outreach, drop-in services, daily meals, emergency beds, an art studio, computer lab and mental health counseling options.

The center can also connect youth and young adults with additional YSS supportive housing programs, such as Lighthouses, Transitional Living Program, and Rapid Re-housing.

Assists the needs of people living on the streets (outside, abandoned buildings, in cars, etc.) in the Omaha Metro area who are interested in connecting with housing and/or other services.
Services provided are:
-Screening and referral for housing programs
-Information and referrals for mainstream benefits (SNAP, Medicaid, GA, etc.)
-Emergency supplies (food, water, blankets, hygiene items)
-Connection to emergency shelter, if desired
-Case Management
-Basic health assessments and connections to long-term mental and primary healthcare

Hope On The Streets reaches out to homeless or near homeless individuals in the downtown Des Moines area and provides nutritious snacks, peer counseling, encouragement and referral information.

Provides a variety of resources to help individuals struggling with or living in homelessness, access healthy food and warmth during cold weather, and essential hygiene and safety supplies. Supplies provided include food, emergency funds to go towards emergency lodging, sleeping bags and mats, and survival backpacks when available.

Works to support unsheltered residents in Champaign County, through regular engagement where ever the residents may be sleeping and/or taking referrals from local agencies. Services focus on meeting the immediate needs of the client such as food, blankets, clothing, toiletries, transportation, housing referrals, and referrals to other agencies for help with vital documents, LINK/Medicaid help, domestic violence, and mental/physical health services.

To provide 3rd party verification of homelessness, CTSO must make contact with the person at the location of homelessness. No limit on referrals, but can only support a limit of 50 at a time.

**May take up to 1 week to respond to a referral.

Street-based outreach services runaway youth and youth experiencing homelessness, near-homelessness, or street dependency.

Drop-in center services include food, basic first aid and hygiene supplies, advocacy, education and information, and referrals to appropriate community service providers. A nurse is onsite three days per week and offers testing for sexually transmitted diseases.

Mental health services include screening, initial intake, therapist and psychiatrist appointments, and arrangements to obtain medications. Referrals are made for housing, supplemental income, and medical treatment.
