Eligibility for traditional program: Youth, ages 0-21, with behavioral concerns, diagnosed with a serious emotional disorder, at risk for out-of-home placement.

Eligibility for transition age program: Young adults, ages 17-25, diagnosed with a serious and persistent mental illness, at risk of entering, or have entered, the adult emergency system.

Services include mental health evaluations to screen, diagnose, and plan treatment for individuals experiencing acute or chronic psychiatric issues. Psychiatric case management is available to assess needs, coordinate services, and monitor progress to ensure effective care.

Integrated Health and Community Outreach Programs are offered, including Jail Diversion programs, Integrated Health programs, and Support Community Living programs. Life skills education is provided to help individuals develop the knowledge and skills necessary for independent living, addressing areas such as job search, money management, nutrition, home management, and health care.

Provides comprehensive, team-based "wrap-around" support services for young adults who have a mental illness. The program assists individuals transitioning from the youth service system to the adult service system and facilitates the return to the community for individuals who have been hospitalized for treatment of acute or chronic mental or emotional disturbances.

Services focus on skill development, self-sufficiency, and community integration through employment and education. Support includes clinical therapeutic services, medication management, daily living skills assistance, and home-based options for individuals unable to leave their homes due to the severity of their mental or emotional conditions or other complicating factors.
