Provides information and referrals to area agencies that offer support services to the Washington and Marion County residents.

Assists Nebraskans who are 60 years and over or have a disability and/or their representatives in accessing services and supports to allow the person to live as independently as possible in the setting of their choice. ADRC staff are available by phone or in person to assist in accessing resources based on the person's needs and preferences. This may include providing information and referrals for services such as housing, personal care, transportation, food and nutrition, public benefits, therapies, employment, etc.

Offers a Disability Access Point, or "DAP", which help people with disabilities, their families, and their caregivers find the services and support they need-both short-term and long-term. DAPs help by giving information, making plans, making referrals and checking in to make sure people are getting the right support to live healthy, independent lives.

Can help with the following services:

- Information and Assistance - based on presented and identified needs, provides information about programs and services, and helps to connect individuals to service providers in their communities.

- Options Counseling - provides guidance to individuals so that they may make informed choices about supports and services, typically over a period of 90 days or less. This includes benefits screening and application assistance for programs and services and follow-ups to make sure supports and decisions are assisting the individual.

- Financial Assistance - provides gap funding and service coordination for programs and services that support individuals and their caregivers so that individuals may live in the home and community of their choice. Help with short-term services and long term services are also provided.

Short-term supports (time-limited) include adaptive and assistive equipment, home and vehicle modifications, peer and parent support services, rent assistance, transportation assistance, and other basic needs (not covered by insurance).

Long-term services and supports includes adult day programs, consumer-directed attendant care (CDAC) (non-skilled and skilled), day habilitation, employment like job skills training, prevocational services, and supported employment, intensive residential services (IRSH), personal response systems, respite services like in-home, in-facility, and day camps, supported community living (SCL) and home-based habilitation (hourly), and residential living settings.

LifeLong Links-Aging & Disability Resource Centers (ADRCs) provide specialized information, referral, and assistance regarding programs and services available for older adults and persons with disabilities. ADRC Options Counselors provide benefits counseling to help empower individuals to make informed choices about their long-term care options and assist them in accessing needed services. Grandparent support program for anyone 55+ who has stepped in to take responsibility for a relative child(ren) in situations where the mother or father cannot.

Offers counseling, information, training, resources, and support as grandparents navigate emotional, educational, technological, and disciplinary issues unique to the child(ren).

Services include legal information relevant to caring for a relative child, as well as possible access to respite care.

Housekeeping and supportive services to include grocery shopping, cleaning, laundry, transportation and personal care.

Family Caregiver Support for those caring for an aging adult or grandparents caring for a minor child. Provides information, assistance, support group, respite care, and supplemental services.

Home delivered meals are provided to homebound by reason of illness, disability or isolation. The meals meet 1/3 of the daily dietary recommended needs of adults 60 years and over and the requirements of the Older Americans Act and state and local laws.

Public benefit services, legal assistance, education, Medicare filing, and volunteer training through SHIIP.

Pediatric Developmental Screening
Offers information on community services. Individuals will speak with an Information and Assistance Specialist who will assess their needs, answer questions, explain what programs are available, and assist them in securing the assistance they need.

Provides a wide variety of independent living services for individuals with disabilities with the objective of helping them to function more independently in family and community settings and to secure and maintain appropriate employment. Services may include information and referral services, referrals for attendant care, attendant recruitment and training programs, independent living skills training, assistance in meeting housing and transportation needs, peer counseling, transition from institutions into the community, deflection from institutional placement, transition from youth to adult services, and advice and guidance regarding legal rights and benefits eligibility.
