Assists Nebraskans who are age 60+ or have a disability (and/or their representatives) to allow the person to live as independently as possible in the setting of their choice.

- Assists in accessing resources based on the person’s needs and preferences, including information and referrals for services such as housing, personal care, transportation, food and nutrition, public benefits, therapies, employment, etc.

Helps seniors, individuals living with a disability, and their families by providing referrals to organizations and agencies for needed services and programs.

Offers 24-hour intakes by phone at the local number and the state hotline. A caseworker will make a home visit to investigate any allegations of abuse and assist the victim. Interventions will be offered to the victim and referrals made to the appropriate organizations. Interventions can include but are not limited to in-home services, legal, medical, relocation, and counseling.

Investigates allegations of abuse, neglect, and exploitation of older adults age 60 and older and persons with disabilities (ages 18-59) who live in domestic (non-institutional) setting. Appointed by the Illinois Department on Aging to investigate allegations of physical and emotional abuse, financial exploitation, confinement/neglect, deprivation of food or medication, sexual abuse, and self-neglect

Provides public health services to the residents of Boone County. Services include blood pressure screening, case management for the Frail Elderly, communicable disease control, care for yourself program (cancer screening program), emergency preparedness planning, immunization clinics, communicable disease control links, and Iowa Department of Public Health Centers for Disease Control and Prevention.

Provides older adults and their caregivers with vital information on available programs, services, and benefits. Professionally trained staff assists individuals with navigating a wide range of resources, including healthcare options, financial aid, and support services. Seniors and caregivers can receive personalized guidance to address their needs and concerns.

Serves as the area's Aging and Disability Resource Center and Area Agency on Aging.

Public health works with older adults and Elderbridge Area Agency on Aging. 

Assists older adults to remain independent in their home as long as possible by providing an evaluation of the older adults' needs, developing a plan of care to meet those needs, and monitoring their plan to make changes as appropriate to keep them independent.

Care coordinators also provide an evaluation in the hospital or at home to educate individuals about their resource options prior to entering a nursing home.
