Provides services coordination/case management to those disabled and/or older adults who meet nursing facility level of care for the Medicaid Waiver Program. This program is based upon the belief that people with care needs should have options for receiving services. Potential participants will meet with a staff person to determine needs and develop a care plan. Eligible persons are offered the choice of receiving home and community-based services or nursing facility care.

The Waiver allows Medicaid money to be used to purchase the following services that are not usually considered 'medical': Personal Care, Companion Care, Chore Services, Adult Day Services, Independence Skills Management, Personal Emergency Response System, Respite Care, Assisted Living Services, Assistive Technology and Home Modifications.

Offers to assist older adults with multiple long-term care needs who wish to remain safely and comfortably independent in their own homes. Services include a comprehensive assessment of need, care planning, and the ongoing assistance of a case manager to initiate and monitor home and community-based services.

Free support services for those with eating disorders, their friends and family.

Committed to empowering persons with disabilities to achieve full participation in community life through informed choices.

Offers families and professionals information and referrals related to the health of children and youth with special health care needs.

Support group for anyone in the general public who has diabetes.  Meetings held in the Conference Dining Room in the lower level of FHN Memorial Hospital in Freeport, unless otherwise noted.

Provides in and out-of-town transportation within Benton County and to surrounding counties for the elderly, handicapped and the general public.

Provides disaster assistance with mobile feeding canteens, spiritual support services, and basic needs provisions. Can also help with cleaning supplies when available.

Offers sober living for those in recovery. Services also include permanent shared housing for adults who are recovering from substance misuse or co-occurring, leaving a treatment program, unstable housing, or reentering the community. 

Provides accurate information on adoption, and lifelong support, so the family is successful in meeting the child’s best interest by offering the following services and resources:

Provides free and confidential pregnancy counseling for families, every step of the way; before, during, and after the birth of the child.

Assistance with making an adoption plan or parenting plan.

Parenting education. Support and education for baby's father on request.

Home studies for prospective adoptive parents.

Newborn care home for baby as a safe haven.

Provides non-perishable and perishable food items once per month, any day of the month the pantry is open, to the residents of the Perry School District. 

Provides non-medical in-home support to the elderly and individuals with disabilities. Services can include menu planning, budgeting, shopping, child care, meal preparation, laundry and general household management including light housekeeping.

Provides public housing units and Section 8 housing choice vouchers for use in the community are provided for low to moderate income families, seniors, and people with disabilities. The primary focus is to provide and maintain quality, affordable housing within a safe, stable environment. In addition, the Authority promotes educational, employment, economic development, and self-sufficiency programs and initiatives for the residents to assist them in enhancing and improving their lives.

Provides a matching fund program to help with utility bills for customers of Ameren Illinois only.
