Supports adults with intellectual and developmental disabilities to find jobs in the community.

-- Discovery: Helps to learn about a client's strengths and interests through observation, conversation, and spending time with clients to help find jobs that fit their goals. Family and friends are also interviewed to learn about a client's likes, dislikes, and strengths. Two home visits and a community visit are conducted.

-- Job Development: Employment Specialists teach social and communication skills needed to be successful at work. They support with touring businesses, job shadowing, and finding a job match. Also provide help creating a resume to give to potential employers. May begin with volunteering so that clients can gain experience and grow skills.

-- Employment Support: After a client is hired, Employment Support develops accommodations and supports for the job. Employment Support forms a relationship with the employer and supports the client to expand job roles. Provides initial job coaching for 2-4 weeks.

Partner Abuse Intervention counseling for those who have been abusive in current or past intimate partner relationships.

Caring Dads/Nurturing Moms, counseling for parents (biological, step, or common law) who have exposed their children to domestic violence, either as witnesses to the abuse of the other parent, and/or as victims themselves.

North Fayette Valley Community Coalition

Provides mentoring services to youth. Program matches students in the North Fayette Valley Community School District with an adult mentor. The focus is to provide youth with positive adult role models and reduce drug and alcohol use among youth through the development of a relationship with a caring adult. Adult mentors are asked to spend four to six hours per month with their youth mentee pursuing activities that they choose together. In addition, the program typically offers a group social/recreational activity for all participants every other month.

Mentors are over 18 years old and have completed a series of background checks. In addition, all mentors receive an initial two-hour training before being matched with a youth and are provided with subsequent trainings throughout the year on a variety of topics.

UIC Division of Specialized Care For Children - Springfield Regional Office

Guides families through their child’s journey with a medical condition. Works with doctors, schools and community groups to create a seamless support system.

Care coordination team works together with families to develop a plan of care that addresses a child’s medical, social, behavioral, educational and financial needs. Care coordination is free for all children who have eligible medical conditions, regardless of their family’s income level.

Care coordination can include finding specialized medical care, helping families understand their child’s diagnosis and medical treatment plan, explaining insurance benefits and attending school meetings.

Helps children who are medically fragile remain in their family home rather than a hospital or skilled nursing facility.

Assists with:

- Finding specialized medical care

- Making sure families understand their child’s diagnosis and medical treatment plan

- Helping families understand their insurance benefits and maximize their coverage

- Attending IEP /504 Plan school meeting

- Connecting families with financial support, grants and other community resources

- Preparing for the transition to adulthood
