Senior Care Options program available to older Nebraskans who are considering admission to a nursing home and may be eligible for Medicaid assistance. Clients are screened according to a standardized assessment process developed by the Department of Health and Human Services.
Care Managers arrange for services, which allow older adults to live as independently as possible, for as long as possible.
Housekeeping and supportive services to include grocery shopping, cleaning, laundry, transportation and personal care.
Family Caregiver Support for those caring for an aging adult or grandparents caring for a minor child. Provides information, assistance, support group, respite care, and supplemental services.
Home delivered meals are provided to homebound by reason of illness, disability or isolation. The meals meet 1/3 of the daily dietary recommended needs of adults 60 years and over and the requirements of the Older Americans Act and state and local laws.
Public benefit services, legal assistance, education, Medicare filing, and volunteer training through SHIIP.

Investigates allegations of abuse, neglect, and exploitation of older adults and persons with disabilities who live in domestic (non-institutional) setting.
Once a report has been filed, a caseworker will make a home visit to investigate any allegations of abuse and assist the victim. Interventions will be offered to the victim and referrals made to the appropriate organizations. Interventions can include but are not limited to in-home services, legal, medical, relocation, and counseling.

Additionally, the service provides access to information about privately owned rental housing options available at below-market rates, including government-subsidized housing and low-income/subsidized private rental housing. These housing options may include units developed or improved with public funds, income-based rental housing, and fixed below-market rate rental housing, benefiting low-income households, older adults, people with disabilities, and other special populations.

Toys for Tots Nov 1st - Nov 30th through Bond of Unity (New Beginnings Discipleship Ministries). To qualify you only need to complete the form below. Please know that families are only permitted to register at 1 Toys for Tots location. Signing up at more than 1 Toys for Tots site can disqualify you from being able to receive any toys.
Toys will be available to be picked up at 530 NE Broadway (NE46th), Des Moines, IA on Friday, December 12th 5-7pm and Saturday, December 13th 10am - 2pm.

In order to be able to serve more families we will not offer a "shopping" opportunity. Toys are prepacked based on the information given below and will be given out in a drive-thru format.
