Offers voluntary programs to eligible landowners and agricultural producers to provide financial and technical assistance to help manage natural resources in a sustainable manner. Through these programs the agency approves contracts to provide financial assistance to help plan and implement conservation practices that address natural resource concerns or opportunities to help save energy, improve soil, water, plant, air, animal and related resources on agricultural lands and non-industrial private forest land, with emphasis on market stability and commodity pricing.

Offers MEDICAID (Medical Assistance) to people who are low income and aged (65 and older), disabled (including blindness), women needing treatment for cancer or pre-cancerous conditions of breast or cervix (who have been screened and diagnosed through the Breast and Cervical Cancer Early Detection program and who do not have credible health insurance coverage), children under 21 years of age, pregnant women, parents or other caretaker relatives of children under age 18 (or under age 19 if they are attending secondary school and are expected to graduate before age 19). Medicaid will pay the costs of Health care, Dental care, In-home health care, Nursing facility care or Residential facility care.

Provides a transitional housing program to survivors with dependent children who are currently staying at the domestic violence shelter. Participants of this initiative are able to transition from the shelter to a transitional housing apartment, where they are offered the opportunity to reside for a period of up to two years, during which they are provided with essential support services.

Home health care and hospice provider.

Coordinates necessary medical services and supportive care which may include skilled nursing, social work, home care, and transportation.

Provides hospice or palliative care, and also provides pediatric home care services and spiritual care services.

Provides outpatient mental health services for individuals and families with licensed professionals. Psychiatric services are available.

Community based health center offering adult and pediatric primary care. Provides a medical home for Chestnut clients, especially those with co-morbid mental illness and/or substance use disorders. Services include:

- Basic primary care, preventive care, and prescription services.

- Screenings (cancer, communicable diseases, cholesterol, blood lead, pediatric vision, hearing, and dental).

- Annual wellness exams for all ages.

- Well child exams and immunizations.

- Lab collections.

- Obstetrical and gynecological care.

- Physicals (school, sports, employment).

Offers consultations to evaluate immigration legal options, advise clients on how to proceed, and represent clients throughout the legal process.

Long term residential placement for youth 10-18 years of age. Outpatient substance abuse treatment and therapy services available for youth placed in the long-term residential program.

Behavioral health services for youth 5-18 years of age depending on the program or service.

Volunteer mentoring program for 3rd through 12th grade in math or reading skills.

Family preservation services in Iowa and Nebraska.

Educational programs for professionals working with youth.
