Partnership between a church and UnityPoint Health Trinity that places a registered nurse in a church to provide assistance with health promotion/education, referrals, health counseling, and emotional support for their parishioners.  The nurses address concerns regarding infants through the elderly.   With the nurse’s guidance, individual churches may operate specific assistance programs such as transportation services, grocery shopping, support groups, visitation programs, or respite care.  Parish nurses visit people in their homes, hospitals, and extended care facilities and help with coordination of care.  Also utilizes health ministers.
