Improves the quality of life of individuals by providing training, education and employment programs. SUPPORTED EMPLOYMENT services are designed to provide personalized assistance to individuals with disabilities. Encouragement in independence by helping job-seekers get the skills necessary to find and maintain employment in the community. Program is focused on finding and utilizing individual strengths. Through job coaching and hands-on training, participants learn to understand their abilities and determine which jobs will allow those abilities to shine and go for their full potential. BENEFITS PLANNING enables beneficiaries with disabilities to make informed choices about going to work also supporting them as they continue progress toward self-sufficiency. Team members will work with beneficiaries to provide in-depth counseling about benefits and the effect of work on those benefits. Education about benefits is provides in order to help beneficiaries attain independence and confidence. Team members counsel beneficiaries on Work Incentive Summary, Benefits Summary and Analysis, PASS plan, Impairment Related Work Expenses (IRWE) and resolve benefits issues.

LIHEAP (Low Income Home Energy Assistance Program) provides supplemental help for the winter heating bills November 1-April 30. Those over 60 or disabled may apply for the season starting in October.

LIHEAP Crisis Assistance, when funding is available, may help with disconnect notices, furnace repair or furnace replacement for homeowners. Crisis assistance is dependent upon funding availability and may be available between April-October.

May have some crisis funding available outside the normal LIHEAP months with local funding, call local County Outreach Office for availability of funding.

Special Olympics Iowa

A non-profit organization that uses sports training and quality competition to provide people with intellectual disabilities the opportunity to participate in a larger society where they are accepted and respected. Special Olympics Iowa currently serves 11,000 athletes and sports partners in all 99 counties in Iowa. Training and competition is offered in 23 different sports throughout the year. SPECIALIZED PROGRAMS: Challenge Days are specially designed events using switch activated equipment for athletes with the most severe and profound physical and intellectual disabilities. Unified Sports is the pairing of athletes with intellectual disabilities and those without on the same team. Athlete Leadership Program (ALP) is designed to train athletes in literacy and speaking skills to enable the athletes to serve as public speakers, board members and other leadership roles in the community. Healthy Athletes is a screening program promoting greater overall health and fitness for the athletes. "Young Athletes" is an introductory program offering play day activities and socialization skills for children with intellectual disabilities between the ages of 2 and 7.

SAL Home Child Care Network - Moline

Trains home child care providers to help ensure the highest quality of care is provided by offering free training and courtesy home visits with activities and crafts for the children, CPR/First Aid certification, and professional development opportunities.

Training for child care providers is offered both through SAL Child Care Resource and Referral programs for all licensed and license-exempt centers and homes in the 16-county IL area from the Quad Cities to central Illinois, and through SAL Home Child Care Network Moline, Galesburg, and Peoria offices for licensed home providers who are part of the Network.

Guides families through their child’s journey with a medical condition. Works with doctors, schools and community groups to create a seamless support system.

Care coordination team works together with families to develop a plan of care that addresses a child’s medical, social, behavioral, educational and financial needs. Care coordination is free for all children who have eligible medical conditions, regardless of their family’s income level.

Care coordination can include finding specialized medical care, helping families understand their child’s diagnosis and medical treatment plan, explaining insurance benefits and attending school meetings.

Helps children who are medically fragile remain in their family home rather than a hospital or skilled nursing facility.

Assists with:

- Finding specialized medical care

- Making sure families understand their child’s diagnosis and medical treatment plan

- Helping families understand their insurance benefits and maximize their coverage

- Attending IEP /504 Plan school meeting

- Connecting families with financial support, grants and other community resources

- Preparing for the transition to adulthood
