Hospice of Dubuque
Provides physical, emotional and spiritual care for seriously-ill individuals, their families, and their caregivers. Also offer emotional and spiritual support to the patient and their loved ones. Individualized, person-centered care is provided by an interdisciplinary team that consists of physicians, nurse practitioners, nurses, social workers, counselors, therapists, and certified nursing assistants. On-call nurses are available 24 hours per day, 7 days per week to answer questions, address needs or concerns, and make home visits. To further support patients and families, Hospice volunteers are available for periodic caregiver respite and companionship. Hospice of Dubuque ensures that patients receive the necessary and appropriate medications, equipment, supplies, and interventions.

Through a central contact person, Services Coordination helps families of children with special needs (below age three):
- Find services to meet developmental educational, financial, health care, child care, respite care, and other needs
- Link with needed services
- Work with multiple providers to make sure services are provided as needed
- Become coordinators of services for their own children in the future

LIHEAP (low income home energy assistance program) provides assistance with a utility bill. Applications are accepted November 1 – April 30; for elderly or disabled individuals applications are accepted starting October 1. Income must fall within 200% of poverty guideline. Call for an appointment. A letter has been sent to those who may be eligible for early application in August or September.
**This program is not designed to pay a household's total energy costs. The program will provide supplemental assistance based on several factors. This is a one-time payment to your energy provider.
