Individualized support is provided in the client's home or other community-based settings and is available 24 hours.  Home-Based Habilitation Services (HBHS) are designed to assist individuals with serious and persistent mental illness with the acquisition, retention or improvement in skills related to living in the community. Services assist in the daily living needs of the client and can be provided 24 hours a day. Community Support Services (CSS) provide integrated services and supports to persons with a serious mental illness. Interdisciplinary teams provide assessments and service planning, monitoring of mental health symptoms, medication management, problem-solving assistance, coordination of appointments and transportation and support.

Our Children's Homestead

Children’s Homestead services the more difficult foster care cases in DCFS. The children are placed into homes with foster parents who are trained and committed to provide care. The child is assigned a case worker who provides case management. The child is teamed with a licensed therapist who works with the child and foster parents to engage the child in programs like a therapeutic mentor program to overcome behavioral and socialization issues.

Nishna Residential Services

Residential services include Traditional Group Homes, Community Support Program, and Supported Community Living Services.

Traditional Group Home offers training in self-help skills, household management and leisure-time skills. These programs integrate people into the community in the least restrictive environment. All individuals are encouraged to have an outside activity. Group home sites are in Red Oak, Malvern, and Shenandoah.

Community Support Program teaches independent living skills to persons living in their own apartment or home. Individuals receive additional support services as needed such as transportation, personal shopping, meal planning, socialization, self-help skills, household management skills, and money management.

Supported Community Living Program provides up to 24 hours of support to assist individuals with their daily needs.

Respite Care Program provides short-term relief in the absence of the family or caregiver, such as a vacation or a night out.
