Offers voluntary programs to eligible landowners and agricultural producers to provide financial and technical assistance to help manage natural resources in a sustainable manner. Through these programs the agency approves contracts to provide financial assistance to help plan and implement conservation practices that address natural resource concerns or opportunities to help save energy, improve soil, water, plant, air, animal and related resources on agricultural lands and non-industrial private forest land, with emphasis on market stability and commodity pricing.

Offers MEDICAID (Medical Assistance) to people who are low income and aged (65 and older), disabled (including blindness), women needing treatment for cancer or pre-cancerous conditions of breast or cervix (who have been screened and diagnosed through the Breast and Cervical Cancer Early Detection program and who do not have credible health insurance coverage), children under 21 years of age, pregnant women, parents or other caretaker relatives of children under age 18 (or under age 19 if they are attending secondary school and are expected to graduate before age 19). Offers a full range of general counseling and psychotherapy services including individual (adult, child and adolescent), group, marital, and family counseling for individuals with emotional, relationship, and life adjustment issues. Counseling is holistic in approach, integrating mind-body-spirit for healing. Include intensive psychotherapy (depression, stress, self-esteem, marriage, divorce, family, and grief counseling); psychological testing and evaluation; consultation to businesses, educational and religious institutions and counseling for pre-school through high school age youth and their families. Primary Health Care - Centralized Intake
Primary Health Care Centralized Intake is the centralized intake location for Polk County. Anyone experiencing homelessness can be referred to PHC for assistance. Each case manager will complete an intake with the household and may provide referrals to shelter, housing, public benefits or any other resource that will meet the household needs. Anyone who may be seeking emergency shelter for their family should come to PHC to complete an intake as all emergency family shelters placements are referred through PHC.
Also offers a shower to those who are homeless and in need.
