First time applications are to be signed in front of representative, then Postal Service will forward applications to proper authorities to be issued. Renewal forms available for clients to fill out and send in themselves. PLEASE NOTE: All minor children (regardless of age) must appear in person when applying for a passport. Children under the age of 16 years must be accompanied by both parents or the child's legal guardian. Notarized consent is required from the non-applying parent if only one appears with child under the age of 16.
